Rules for receiving and considering complaints and requests

The patient has the right to submit a complaint or request:

• in writing to the following address: Independent Complex of Public Open Healthcare Facilities Warszawa-Ochota, ul. Szczęśliwicka 36, 02-353 Warsaw
• orally into a record signed by the person submitting the complaint or application and the employee of SZPZLO Warszawa-Ochota receiving the application. Complaints and motions submitted orally to the minutes are then immediately forwarded to the secretariat of SZPZLO Warszawa-Ochota and registered
• via e-mail to the following address: sekretariat@szpzlo-ochota.pl
• by fax at (22) 823-12-19
• by telegraph or teleprinter

An employee of the SZPZLO Warszawa-Ochota receiving a complaint or application is obliged to confirm the submission of the complaint or application if the person submitting the complaint or application so requests.
The Director receives complaints and requests once a week, every Monday from 12:00 to 14:00.

Consideration of complaints and requests

1. In the Independent Complex of Public Open Healthcare Facilities Warsaw-Ochota, the tasks of the Commission for Receiving and Coordinating the Processing of Complaints and Applications, as well as preparing draft responses to complaints and applications, are: consideration of complaints and requests.
2. Complaints and applications that do not contain the name and surname (name), postal address and signature of the applicant will not be considered.
3. In the case of a complaint or application submitted electronically, if it does not include a postal address, the Commission for The Office for the consideration of complaints and applications requests the complainant or applicant to supplement the missing postal address within 7 days, instructing them that failure to remove these deficiencies will result in the complaint or application not being considered.
4. If the subject of the complaint or application cannot be determined from the content of the complaint or application, the person submitting the complaint or application is requested to submit an explanation or supplementation within 7 days of receiving the request, with the instruction that failure to remove the deficiencies will result in the complaint or application not being considered.
5. If consideration of a complaint or application requires prior investigation and clarification of the matter, the Commission for for the consideration of complaints and applications shall contact the head of the relevant department or organizational unit of the Team or the employee concerned by the complaint in order to obtain the necessary materials and explanations.
6. A complaint concerning a specific person may not be forwarded for consideration to that person or to a person to whom that person is in a relationship of superiority.
7. The response to the complaint or request, after being signed by the Director or an authorized person, is sent by registered mail with acknowledgment of receipt.
8. Complaints and requests are considered immediately, but no later than within one month from the date of submission.
